How do I register with NAME OF DPC PROVIDER to gain access to their services? Click here to learn how to get registered today and gain access to your personalized NAME OF DPC PROVIDER Portal.

What can the Apta Concierge Team do for me? Apta Concierge is your single point of customer support, and a new customer service phone number. You will receive an updated ID Card and an Akimbo Cash Pay Card. NAME OF PROVIDER is your preferred, no cost, direct primary care provider. Apta Concierge will work with NAME OF PROVIDER to manage all specialty care. Apta Concierge will coordinate specialty care and contact you with instructions. Copayments and Deductibles are waived when you work with Apta Concierge. You will register as self-pay for care and follow Apta Concierge’s Payment Instructions: - Pay for low-cost specialty provider visits using your Akimbo Cash Pay Card, submit receipts from payment to Apta Concierge within 24 hours of payment. - Apta Concierge will pay in full for expensive care, such as procedures and surgery, on your behalf, prior to your date of service.

Who do I contact if I need a replacement ID card or haven't received one yet? When you sign up for health insurance, you will be sent a member identification (ID) card to your home address. You can request additional cards by contacting your Apta Concierge Team at (XXX) XXX-XXXX. Your member ID card has Health and Pharmacy Plan information that you and your Healthcare providers will use. Call the Apta Concierge Team when you: - Have questions about your benefits - Need help finding the right doctor - Need a medical procedure and want to save money - Need help with an unexpected bill after a procedure - Need direction with pre-certification for a procedure

What if I need urgent care or need to visit an emergency room? NAME OF DPC PROVIDER should be your first choice for common urgent care needs, as they can handle most 24/7 urgent care issues. Examples include but are not limited to: - Common cold - Rash - Sore throat - Headaches - UTI’s - Pink eye - Muscle aches If you have a life-threatening Emergency: - Register as self-pay. - Contact Apta Concierge as soon as reasonably possible. Where do I submit receipts from payment to Apta Concierge when I have register as self-pay for care and I pay the provider during visits using your Akimbo Cash Pay Card?

Please submit receipts from payment to Apta Concierge within 24 hours of payment by emailing aptacash@Apta-health.com

  3. Revive Healthcare is your preferred, no cost, virtual primary care provider.
     

  4. Apta Concierge will work with Revive Health to manage all specialty care.
     

  5. Apta Concierge will coordinate specialty care and contact you with instructions. 
     

  6. Copayments and Deductibles are waived when you work with Apta Concierge.
     

  7. You will register as self-pay for care and follow Apta Concierge’s Payment Instructions:

  • Pay for low-cost specialty provider visits using your Healthcare Debit Card, submit receipts from payment to Apta Concierge within 24 hours of payment by texting the receipt to 801-823-4324 or emailing aptacash@Apta-health.com.

  • Apta Concierge will pay in full for expensive care, such as procedures and surgery, on your behalf, prior to your date of service.
